I am going to visit Poland and Czech Republic at the end of this year most likely. I must say I am being pretty addicted and almost disabled if I dont have my mobile phone by my side. I need it for work and I need to stay in touch with my family. I will be some time in Krakow and then in Prague, so I would like to know which phone card I could use for both cities/countries or how that functions?

What not planning to stick with yours, I guess roaming will be expensive right? Well you could use rangeRoamers International SIM card. I think its the most popular one there and it will provide you the availability in any area. It costs $49 and you are getting for start $20 prepaid airtime. No need for contract.
